The COID-19 pandemic has entered into the second wave in the majority of countries and the third wave in a few countries. It has changed the lifestyle and working norms in all industries across the globe.
Now COVID -19 safety has become an integral part of the organization's health and safety system and to ensure its proper monitoring and improvement its regress reporting is indispensable. 
In this article, we will discuss the COVID -19 weekly report for medium-size facilities where the staff size is between10 to 150. The report template is also included in this blog for your use with ease.

Title of the report with report number because it provides or links the history of COVID -19 reporting and consistency

Report Timeline: [Dec 03 to Dec 9, 2020]

Report timeline that includes the dates of monitoring (Composed of 7 days). It helps organization management to understand the COVID -19 trends.

Section # A

STAFF HEALTH SURVEILLANCE STATUS

This section contains details of daily staff /visitors' health surveillance checks that include temperature check, COVID -19 symptoms declaration. This helps an organization with two filters one by checking entrant temperature physically and the second is a declaration of COVID-19 symptoms at the entrance.

Surveillance Parameter

The surveillance parameters include temperature check which must be below 38° C, the entrant must not have fever, cough, body pain, or sore throat. If anyone has any of the said symptoms he/she must consult with an HSE officer or duty nurse before further proceeding to the office/working area.

Remarks columns are there for staff/entrant to share any important information which can help us to ensure staff/visitors safety

Section # B

COVID – 19 SAFETY SOPS NON-COMPLIANCE STATUS 

This section is designed to record all non-compliances observed by the COVID-19 Safety monitors and entered into the online portal @safetysupport. Theses noncompliance can be of the below nature;
  1. Any staff not wearing a face mask
  2. Staff wear face mask but not as per guidelines e.g. nose is uncovered
  3. Staff don’t maintain physical distancing which must be 2 meters or 6 feet between or among others 
  4. Make any physical contact includes shake hands, hugging, or sharing anything physically

Name of Staff

  • Name of staff as written in HR records
  • Department
  • Department or unit name to differentiate among two same names
  • Date of Non-Compliance
Type of non-compliance as mentioned above can be one out of 4 or can be multiple non-compliances

Category of SoP Observation

The category of SoP violation can vary from organization to organization you can select your organization SoPs to fill the category of SoP observation. For example, the common SoPs for COVID -19 Safety is related to the facemask, physical distancing, and handwashing.

Remarks

In this remarks category, any important information about the non-compliant can be mentioned or attached. This category can be used as an evidence recording column in some cases where evidence is needed to prove the non-compliance.

COVID – 19 SAFETY MATERIAL STOCK STATUS 

COVID-19 safety material stock is an important section to know the status of covid-19 safety facilities in office/workplace, the status includes;
  1. Availability of hand sanitizers on the ground and in stock
  2. Availability of facemask on the ground and in stock
  3. Availability of hand gloves on the ground and in stock 
  4. Availability of face shield on the ground and in stock
  5. Availability of temperature checking device on the ground and in backup

  • Stock Status 
  • Details of stock with an appropriate unit

Section # D

IMPORTANT ACTIONS TAKEN 

This section contains all important actions taken in the monitoring week, these actions include physical interventions and awareness messages and alerts. 

Actions list

List down all important actions taken in a week time period that includes;
Physical interventions which can be decontamination or installation of sanitizers
Awareness sessions, emails, and alerts 
Few examples of action can be listed as under;
  1. Sending an email to all staff to update the current status of COVID-19 cases in-country
  2. Sent COVID-19 alert to communicate latest guidelines issued by Govt. health department
  3. Installation of COVID - 19 Safety board at the office to intimate visitors about COVID - 19 Safety SoPs
  4. Formation of help desk for COVID - 19 infected staff
  5. Installation of the thermal camera at the entrance gate to check the temperature of all entrants
